Loop - Clinical Softworks

Loop is a feature-rich app to be used by care teams to refer and communicate with other teams utilizing a patient-centric approach. Loop can be used within your own health system or have access to other care teams outside of your own - it's up to you! The Loop Team will be providing updates over time as we solve for gaps in the referral process as patients traverse the care continuum. A big problem, but something that we have spent a great deal of time studying! We hope you enjoy the app and appreciate any feedback!
