WatchWell

WatchWellTM is the mobile application that delivers assessment tools and plan of care reminders to patients and other care team members from the Synchrony Coordinator platform. All care team members, including family and loved ones and can stay connected with the patient by viewing their daily activities and connecting via video chats and messaging. Medical providers can remotely monitor a patients changing risk through customized assessments that are delivered to the WatchWell app. Compliance is maximized via regular notifications and messaging reminders.
