Reya Diabetes Remote Care

Reya Diabetes Home Monitoring helps nurses and doctors manage their patients on SMBG (Self Monitoring of Blood Glucose) remotely. Eligible patients are onboarded by a nurse on the smartphone app. They use the Patient App to add sugar readings. Nurses are immediately alerted of high or low sugar readings, allowing quick followup with patients. Doctors generate digital prescriptions for the patient remotely. The system enables care continuity and prompt feedback loops between the patient and their care team. Overall, this leads to better patient experience, and improved quality of care. This App has been integrated with Apple Health App to fetch Walking + Running distance data.
